To authorize services, insurers ask for the documents below. Do not worry if you are missing some. Call us and we will walk through it together.
- Referral or Prescription for ABA TherapyFrom a licensed physician, including the ICD-10 code(s) based on the diagnosis.
- Medical Necessity LetterName, date of birth, diagnosis confirmation, and the rationale for ABA as medically necessary.
- Comprehensive Diagnostic EvaluationA diagnostic report from a licensed physician, psychologist, or neurologist confirming the diagnosis.
- Insurance InformationFront and back of all active insurance cards, primary and secondary.
- IEP or 504 Plan, If ApplicableThe most recent plan or similar document from your child's school.
- Previous ABA Records, If ApplicablePrior assessments, behavior plans, and a transfer form when switching providers.
